authorize Living Word Shanghai to contact or obtain official records from any educational institutions
previously attended by the applicant. I understand that LWS reserves the right to alter or reserve any
decision regarding admissions or enrollment made on the basis of incorrect or incomplete in

I hereby agree that I will settle of my child’s fees at LWS according to the school’s Fee Schedule and
Payment Policy. I assume full responsibility to ensure the timely and ultimate payment.
本人会在学校要求的截止缴费日期前缴清学杂费，我同意承担全部责任保证最后付款的及时性。
There may be occasions when LWS or its management or its associated schools would like to use a
student’s image, whether in photographys or video footage, for use in the school’s Yearbook,
newsletters, etc., as well as the promotion of the school through brochures, flyers, application kits,
banners and advertising, etc. I hereby agree to allow LWS or its management or its associated schools to
use my child’s image as outlined above.
上海美高学校或其管理机构或其关联学校在一些场合会使用学生的照片或录影镜头，用于学校、
家长通讯或者学校Yearbook等刊物上。学生的肖像有时也会出现在小册子、传单、申请用品、横
幅和广告等宣传上，作为学校推广活动使用。本人同意贵校或其管理机构或其关系学校使用本人
孩子的肖像作以上所述之用。
